EKG Lead Placement

Placement of the limb leads on a three or five lead system.


A five lead system will use all the leads shown below, whereas a three lead system uses only the black, white and red leads. Two lead telemetry systems use the black and red leads, but placement may change depending on what EKG lead (view) is required.

The black and white leads are placed on the shoulder area where the shoulder connects to the torso. The green and red leads are placed on the lowest rib on both sides of the torso, and the brown lead is placed at the 4th intercostal space, just to the right of the sternum.

 

Addition of the Chest Leads

To make up the full 12-lead system, additional leads are placed across the chest wall. These leads are called V1 to V6.

V1 - 4th intercostal space, just to the right of the sternum.
V2 - 4th intercostal space, just to the left of the sternum.
V3 – Midway between V2 and V4
V4 - Mid clavicular line, 5th intercostal space.
V5 – Anterior axillary line, between V4 & V6
V6 - Mid axillary line, horizontal with V4.
